Not affiliated with the Social Security Administration. We are not affiliated with, endorsed by, or sponsored by the Social Security Administration ("SSA") or any federal, state, or local government agency. We are a private lead generation service. You can apply for Social Security Disability Insurance ("SSDI"), Supplemental Security Income ("SSI"), or any other government benefit directly with the SSA, free of charge, at www.ssa.gov or by calling 1-800-772-1213.

No guarantee of benefits or approval. Submitting information through this Website does not guarantee that you will qualify for, be approved for, or receive any SSDI, SSI, or other government benefit. Eligibility and benefit amounts are determined solely by the SSA based on your individual work history, medical condition, and other factors.

We are a lead generation service — not a law firm or representative. We do not provide legal advice, representation, medical advice, or SSA claims services. Our role is limited to collecting contact information from consumers who request more information about SSDI/SSI services and transmitting that information to third parties who may contact the consumer.

1. Information We Collect

Information you provide to us. When you submit a form on the Website, you provide to us, and we collect, the following categories of personal information:

• Telephone number
• Email address
• Mailing address, including ZIP code

We do not request or collect through the Website: Social Security numbers, medical or health information, diagnosis or treatment information, financial account information, driver's license or government ID numbers, or biometric information.

Information collected automatically. When you visit the Website, we (and our service providers and advertising partners) may automatically collect:

We do not use the personal information you submit through the Website for our own marketing, profiling, or advertising purposes beyond transmitting it to Partners and maintaining the records described above.

3. How We Share and Sell Your Information

Sale and sharing with Partners. We sell and share the personal information you submit through the Website. The primary purpose of this Website is to collect consumer contact information and transfer that information to Partners for a fee. By submitting a form on the Website, you acknowledge and agree that we will sell and/or share the contact information you provide to one or more Partners.

Partners may include, without limitation:

• Marketing and advertising companies
• Attorneys and law firms
• SSDI/SSI advocacy organizations
• Non-attorney representatives authorized to appear before the SSA
• Other lead buyers and lead brokers

A current list of categories of Partners and, where available, specific Partners that may receive your information can be requested by emailing support@ssdiapprovalassist.com with the subject line "Partners List Request."

Partners may in turn share your information with their own affiliates, service providers, vendors, sub-buyers, or downstream marketing partners. We do not control the practices of Partners after your information is transferred. Each Partner is responsible for its own use of your information, including its own privacy practices, communications practices, and compliance with applicable law. We encourage you to review the privacy policy of any Partner that contacts you.

Service providers. We share information with third-party service providers that help us operate the Website, including hosting providers, form processors, analytics providers, lead-verification and consent-recording providers (such as TrustedForm or Jornaya), email and messaging vendors, and fraud-prevention services.

Legal and safety disclosures. Your consent is not a condition of receiving any goods or services. Message and data rates may apply to calls and text messages. Message frequency varies.

Revoking consent. You may revoke your consent to be contacted by a Partner by following the opt-out instructions provided by that Partner (for example, replying STOP to a text message, clicking "unsubscribe" in an email, or asking the Partner by phone to stop contacting you). Because we do not control Partners after your information is transferred, you must contact each Partner directly to revoke consent with them.

5. Cookies and Tracking Technologies

The Website uses cookies, pixels, tags, software development kits, and similar tracking technologies to operate the Website, measure performance, deliver targeted advertising, and analyze how consumers interact with the Website. We do not collect, derive, or infer consumer health data through the Website. Specifically:

8. How to Exercise Your Rights

Submitting a request. To submit a privacy-rights request, email support@ssdiapprovalassist.com with:

• The specific right you are exercising (for example, "delete my information" or "do not sell or share my information")

We may need to verify your identity before processing your request. Verification typically requires matching information you submit with information we have on file. We will respond within the time period required by applicable law (generally 45 days, with one 45-day extension permitted under California law).

Authorized agents. You may designate an authorized agent to submit a request on your behalf. We may require written authorization signed by you and verification of the agent's identity before acting on the request.

Appeal of denied requests. If we deny your privacy-rights request in whole or in part, you may appeal that decision within 60 days by emailing support@ssdiapprovalassist.com with the subject line "Privacy Appeal." Your appeal should include the original request, the date of our response, and the reason you believe the denial was incorrect. We will respond to your appeal within 60 days. If your appeal is denied, you may contact your state attorney general to lodge a complaint where your state law allows.

9. Data Retention

We retain personal information only as long as reasonably necessary to fulfill the purposes described in this Policy. Specifically:

• Lead transmission records. Retained for the time required to transmit your information to Partners and confirm successful delivery.
• TCPA consent records. Retained for no fewer than 5 years from the date of last contact, consistent with the federal statute of limitations under the Telephone Consumer Protection Act and similar state laws. These records typically include the form snapshot, IP address, timestamp, and consent token (such as a TrustedForm or Jornaya/LeadiD certificate).
• Fraud-prevention and legal-compliance records. Retained as long as reasonably necessary to investigate disputes, prevent fraud, and comply with legal obligations.

When we no longer need to retain personal information, we will delete or de-identify it.

10. Data Security

We use commercially reasonable administrative, technical, and physical safeguards designed to protect personal information against loss, misuse, unauthorized access, disclosure, alteration, and destruction. However, no method of transmission over the Internet or method of electronic storage is completely secure, and we cannot guarantee the absolute security of any information you submit.

11. Children's Privacy

The Website is intended for adults. Consistent with the federal Children's Online Privacy Protection Act ("COPPA"), we do not knowingly collect personal information from children under 13 years of age.
